*Parental Waiver Form and COVID-19 Vaccination Required – The Parental Waiver Form and a copy of the attendees COVID-19 Vaccination Card must be emailed or mailed to the Hawaiian Humane Society.  Once all documents are received and approved, the Hawaiian Humane Society will email the parent listed on the Parental Waiver Form an approval to proceed with ticket purchase.
